What is an ACO?

Accountable Care Organizations (ACOs) are groups of doctors, hospitals and other health care providers working together to provide coordinated care to their patients.

Be Involved

Be Accountable
As important as it is to stay involved in your health care, it is equally important to be accountable for that care as well. This includes attending all medical appointments and actively participating in your care and treatments.

Be certain to speak up about your health care concerns and ask questions about your care. It is important that you fully understand your care plan.

Be Patient
The United States has one of the most complex medical systems in the world and changing that system will not happen overnight. Improvements and changes will help improve quality and safety of health care for every patient, but it will take time for the overall effects of these changes to make a difference.

Be patient and know that, as an ACO, OSF HealthCare will continue to pursue its Mission to serve all persons with the greatest care and love.

Participation

  • Beneficiariesdo notparticipate; only providers participate in the ACO
  • Your health benefits do not change.
  • There is no ID card.
  • You may still see any doctors or health care providers you choose, even if those doctors or health care providers are not part of OSF HealthCare or an ACO.

Information

  • The privacy and security of your health information is always protected by Federal Law
  • Your health information is used by your healthcare providers to help improve and coordinate your care.
  • You have the right to decline to share your information.
